About CCM

Connections Case Mgmt (CCM) is a Support Services Brokerage and a nonprofit 501(c)3 agency. Since 2001, CCM has been providing case management to adults with developmental disabilities living in the communities of Coos, Curry, Douglas, and Klamath Counties.
