Kya A. Personal Agent

Kya is a Native to Klamath Falls & surrounding area.  She is an active member in the community & very involved in her Indigenous Peoples Ways and Culture.

She has worked in intensive social services for 10+ years and considers herself both a Natural and Professional Advocate. Serving others, along with helping folks see and reach their full potential, is very important to Kya. She is Passionate about this field and is super excited to be part of the Connections Case Mgmt Team.

One of Kya’s greatest accomplishments is her daughter.  Being a mother and a big time family member, are her biggest blessings. Kya has a big family and is the only sister to a bunch of rowdy (in the best way) brothers, whom are also her best friends.

Outside of work, Kya has many hobbies. She is an avid Oregon enthusiast. She loves hiking, kayaking, hunting, fishing, waterfall and beach exploring, playing ball and flat out getting dirty, as well as cooking, crafting, decorating, reading, watching movies, hanging out with her kitties and all that other fun-to-her stuff.

Leslie T.

Leslie T. Personal Agent

Leslie was fortunate enough to grow up in Coos Bay.  Born from commercial fisherman and loggers, Leslie has a great appreciation and love for the history of Coos County, as well as all bounties the area has to offer.

From blackberry to mushroom picking, to fishing, swimming up Powers, or looking for agates on the beach, Leslie feels there is no other place she would rather live and raise her daughter in.

Leslie has worked in social services for over 3 years with a variety of people from all different backgrounds.  She is excited to start her new journey with CCM and looks forward to making a positive impact.

Mary Lee L.E.

Mary Lee L.E. Assessor Specialist

Mary Lee was born and raised in Klamath Falls.  She has a Bachelor of Arts degree from University of Portland and has been with Connections since 2014, first as a Personal Agent and now as an Assessor Specialist.  She loves working with our customers and is always learning from them.

Mary Lee lives in Klamath Falls with her husband, their two amazing daughters and several four-legged family members.

Mendi H.

Mendi H. Personal Agent

Mendi was born and raised in Arizona and moved to Oregon in 1995. She has many years of experience in the I/DD field working  in local group homes as well as coaching bowling and managing medicals for Douglas County Special Olympics.  In July 2019, she began working at Connections as a PA.

Mendi has 4 grown sons and is proud to say that she is a grandma of 3 amazing grandkids. She has 2 cats and 1 dog. She loves all animals, big and small. Her favorite holiday is Christmas, where she goes a little crazy with decorating her home and office. In her off time, you can find her around her goldfish pond in her yard or up in the mountains mudding, hunting, fishing, hiking and camping.

Robin M.

Robin M. Brokerage Director

Family member and community advocate for persons with I/DD for 27+ years.

Founding member of W.I.S.H. a 501(c)3 to establish appropriate and affordable housing for persons with I/DD.

Past member of the Staley Implementation Group.

Original member of the local Self-Directed Supports (SDS) group, the precursor to the establishment of Brokerages in Oregon.

Lives in Roseburg, Oregon with her husband and dog, Trigger.

Robyn R.

Robyn R. Program Coordinator

Robyn was born and raised in Grants Pass Oregon, spent 7 years in Coos Bay before landing in Roseburg in 2001.  She has been in the I/DD field for over 30 years, working as direct support, managing group homes, agency behavior specialist and OIS mentor/independent instructor.

She enjoys volunteering in her community, participating with the Dance committee in Douglas County (since 2002) and coaching Special Olympics. Robyn is married with four adult children and blessed with three beautiful grandkids.  She loves photography, nature, plants, animals, and live music.

Ruthie C.

Ruthie C. Assessor Coordinator

Ruthie moved to Oregon in 1995 after graduating from the University of Chico State and began her social work career.  Before becoming a Personal Agent for Connections in 2012, she worked as a Case Manager for Pregnant & Parenting Teens in a Medford High School, a High School Special Education Teacher in Hermiston, and a Social Worker for Child Welfare in Grants Pass.

Ruthie has two adult children, Dominic and Demitri, the latter is serving our country in the US Army.  She is passionate about people and frogs.  After all, one may someday turn into a prince. In her free time, she loves cycling and crafting activities.  She is also grateful to God for the mountain of friendships she has been blessed with in her lifetime.

Shantell B.

Shantell B. Personal Agent

Shantell was born in Roseburg but lived in Arizona for most of her childhood while her father served in the United States Marine Corps. In 2014, she graduated from Oregon State University with a Bachelors in Human Development and Family Sciences. Before coming to Connections Case Management, she worked as a case manager for the county’s mental health and disability services programs. In her free time, she enjoys painting, crafting, and spending time with her family and animals.

Staci B.

Staci B. Personal Agent

Staci has lived in Douglas County since 2006. She has spent the past twenty years serving in various social services positions. She is excited to start her career at Connections Case Mgmt.

Staci has two amazing adult daughters and two incredible granddaughters. She also has four fur grandsons and two fur babies of her own.

In her free time, you will find her surrounded by family & friends at the beach, on the lake fishing, or exploring the beautiful outdoors. She also enjoys reading suspense novels and baking treats for her loved ones.

Tawny W.

Tawny W. Provider eXPRS Specialist

Residing in Roseburg since 1998, Tawny and her husband love working with people.  She is the Director of Worship at Garden Valley Church and loves leading her team, as well as, helping her husband with the Jr. High Youth Ministry.  Nothing keeps you on your toes like working with teenagers, or as she calls them…tweeners.

Tawny loves seeing people live their lives to the fullest. She loves to sing, read, and hang out with friends. She has worked at Connections since 2010 & enjoys serving everyone who comes into the office.
