Who We Serve
Connections Case Mgmt (CCM) provides case management to adults with intellectual and developmental disabilities who live in their own home or family home. Since 2001, we have been supporting individuals living in Coos, Curry, Douglas, and Klamath Counties.
We refer to the individuals we support as customers rather than clients. Customers are at the heart of what we do, always.
Eligibility
To be eligible for services through CCM, you must be an adult (age 18 or older) residing in Oregon, with a confirmed diagnosis of a developmental or intellectual disability, living in your own home or family home, and not currently receiving residential services.
How do I become a CCM customer?
Customers only come to CCM through a referral from a Community Developmental Disabilities Program (CDDP).
- If you are already a customer at a CDDP, you can request to transfer your case management to CCM at any time.
- CCM and the CDDP will coordinate a transfer date, if capacity is available.
- After a transfer date is established, CCM will call you to schedule an entry meeting. At this meeting, you will meet your PA and begin planning for the life you want!
- If you are not a CDDP customer and want to apply for DD Services, contact the CDDP in your area. Upon determination of eligibility for DD services, you can request a referral to CCM from the CDDP.
